I suffer from Patella tracking disorder and it is indeed a case where the 
knee cap slides up and down in the groove but rubs against the edge because 
it isn't held in place properly.  This can be very painful, especially 
during distance hikes obviously.

The fix was a knee brace with a "patella sleeve" or "patella window" that 
holds the knee cap in place as you walk.  The long term fix is hours in the 
gym strengthening the muscles around the patella so that they can hold the 
knee cap in place without the brace.  Usually it's cause by having outer 
muscles stronger than the ones on the inside of the knee.  Ask your local 
physical trainer/doctor for specific exercises.
