The Earth is normally about 50% cloud covered, with some areas being cloud 
covered a lot more than 50% and others being cloud free most of the time.  
It's also a little difficult to catch the entire Earth in total darkness 
long enough to take that kind of shot  :-)

The answer is that it's a 'composite' picture, from hundreds of pictures 
taken over a long time period.  We started doing this kind of thing back in 
the 60's.  The technology has improved greatly since then.
