Simply contact the customer service number on your credit card.  They're the
one who will process a credit.  A form will be sent to you and you need only
to cite the circumstances of the deal.  They'll issue you a credit, and it
becomes the responsibility of the vendor to contest the credit.  Likely
under these circumstances, they won't contest.

This works equally well of purchases you've made and have been unable to
receive a good resolution.  Most companies don't want the bad press and
they'll be cooperative.  But if you feel like your back is up against the
wall, your credit card company will assist you.  A credit to your account,
slapping the vendor in the wallet, is, by far, the fastest way to get the
attention of any customer service department.
