Ideally, the bottom of your pot is just above the hottest part (blue) of 
the flame. With a clear flame, this can be hard to see.

Yes, along with many other euphemisms.

The perlite provides a larger surface area for the ethanol to volatilize 
and burn. More flame, more heat. It can also help contain the alcohol if 
you bump the container, and avoid a flaming splash on yourself.
