rejection becomes an issue several months after a transplant? Another 
transplant? Medication? ???? "

Unfortunately, this is not all that uncommon. It is unlikely he will 
require another transplant. Having one transplant makes him more likely to 
reject a second. There will be changes in his medications and changes in 
lifestyle, such as reducing stress of his tour and such. This is only a 
bump in his road, not a disaster.
