  Thanks for the links.  The boots are fairly old; but I've had this problem intermittently since I got them.  It seems not to matter what sock combination I use, or which way I lace the boots, unless I lace them really loose at the top--which kinda defeats the purpose of having a top.  I don't think my heel is moving--I definitely *would* get heel blisters if that were the case. I've got bony heels too ;-)

 I guess the first question is; how old are the boots? We're talking apples
and oranges here, but I had that problem with a pair of ski boots that were
just getting old, and the liners were shot.

Second point is that there are various ways to lace your boots that may or
may not help..

These are just a couple of hits that I found but haven't checked out in
detail. You may want to pay some attention to whether or not your heel is
lifting inside of the boot. This would normally cause heel blisters, but if
your foot is moving inside the boot it obviously causes changes in the way
the boot is fitting and could cause something like your ankle problem.
Last but not least is MoleFoam!!!

Sorry I don't have a better answer, but I'm not a boot fitter... just have
my own bony feet to deal with!
