There's a good article titled "Germs can lurk in water bottles" in today's Boston Globe:

It discusses biofilms, the problems they cause, how they can clog filters and how to collect water to (mostly) avoid picking them up.  It also mentions that the primary rout of intestinal infection on the trail is not water, but not washing hands after going to the bathroom and handling food.
