Mea Culpa is Latin for "my fault". It is part of the prayer "Confiteor" ("I
confess") in the Catholic Mass (when it was in Latin). The complete phrase
is "mea culpa, mea culpa, mea maxima culpa" usually translated "through my
fault, through my fault, through my most grievous fault".
