It contains pressurized CO2, or other gas.  I'm not sure what Guinness uses.

When the can is opened its interior pressure is reduced to the atmospheric
pressure.  The CO2 in the little ball is now higher than the beverage. So
the gas bubbles through very little holes creating the foamy head, somewhat
like a true draft.
